Degenerative Cervical Spinal Pathologies and Current Approaches to Treatment Options

It is especially seen in the elderly population. More than 70% of individuals over 65 have pathological or radiological manifestations of cervical degenerative disease. The clinical presentation of DSM can be defined as a syndrome associated with upper extremity dysfunction (clumsiness, lethargy, paresthesia, etc.), lower extremity findings (imbalance, fatigue, lethargy, etc.), and bladder dysfunction (sudden urinary incontinence). Magnetic Resonance Imaging is the gold standard for DSM diagnosis. Computed tomography imaging is essential in determining ossification and in surgical planning. Cervical CT is an indispensable imaging method for planning an anterior, posterior, or combined approach. The primary differential diagnoses include amyotrophic lateral sclerosis, multiple sclerosis, normal pressure hydrocephalus, and intracranial diseases. Surgical treatment for DSM disease is the primary treatment option for patients with progressively worsening balance and skills. With the surgical approaches that can be applied in an anterior, posterior, or combined manner, the main goal should be to open the spinal cord, correct the cervical physiological lordosis, and provide stability.
