Recent reports indicate that these arteries frequently have substantial arteriosclerotic changes in the older age group, when cerebral revascularization is usually indicated. These pathological changes could potentially reduce the efficacy of the procedure.
An alternative approach is to use a Saphenous Vein Graft (SVG) for such patients include occlusion or severe disease of the Common Carotid Artery (CCA), high-grade stenosis or occlusion of the extra cranial arteries, or the need for an immediate high-flow bypass.
In this report, we describe our initial experience with the use of interposition SVG between the CCA and Middle Cerebral Artery. The indication, result and technical aspect of the operation are reviewed.
