We performed optic nerve sheath fenestration in five patients (six eyes) who had continuing deterioration of visual function despite maximum medical treatment. Symptoms including headache and diplopia regressed rapidly in all cases. There were definitive improvements in visual field defects and visual acuity. Papilledema also disappeared gradually in all cases. Except for one patient, all fellow eyes also showed improvement after unilateral operation. No eye was reoperated. Our results provide more evidence to the fact that optic nerve sheath fenestration can successfully forestall the deleterious effects of pseudotumor cerebri on the optic nerve and bring symptomatic relief.
