Sixteen patients who underwent transsphenoidal microsurgery between 1987 and 1996 for prolactinomas were studied in order to evaluate surgical and endocrinological results. The mean preoperative serum prolactin levels were 165.5 ng/ml in patients with microprolactinomas, and 800 ng/ml in patients with macroprolactinomas. Normalization of serum prolactin levels (<25 ng/ml) occurred in 7 of 9 patients with microprolactinoma, but in 3 of 7 patients with macroprolactinoma, the prolactin levels lowered significantly without normalization. Remission of amenorrhea-galactorrhea syndrome occurred in 9 of 11 women. This study suggests that high level of serum prolactin restricts the success of transsphenoidal surgery.
