An infraoptic course of the proximal anterior cerebral artery is a very rare anomaly. A 34-year-old rnale patient was admitted on the second day of a subarachnoid hemorrhage. Neurological examination was normal except for mild neck stiffness. Angiography revealed a low bifurcation of the right internal carotid artery, fılling of the distal part of both anterior cerebral arteries by an anomalous Al tract arising from the origin of the right ophthalmic artery, and an anterior communicating artery aneurysm. The aneurysm was clipped. No complications occurred after surgery. The anomalous infraoptic proximal tract of the anterior cerebral artery was documented by magnetic resonance angiography after surgery. The need for a careful angiographic evaluation was emphasized for surgical treatment of anterior communicating artery aneurysms associated with these anomalies.
