Peroneal neuropathy may develop following loss of more than 15% of the body weight; however this is a rare cause of peroneal neuropathy. The peroneal nerve is often exposed to external pressure because of its anatomical course. Pressure most frequently occurs at the caput of fibula. Positional causes such as external compression, trauma, crossing the legs, and squatting may result in peroneal nerve trapping. This situation requires careful clinical, radiological, electrophysological evaluation and a careful treatment plan. In the present study, bilateral foot drop following extreme weight loss has been reported. Bilateral peroneal compression neuropathy following weight loss in a 29-year-old woman has been presented along with electrophysiological and MR Neurography findings and the management details are reviewed with the literature.
