In this report, we present two hemorrhagic stroke cases caused by cocaine. The first case is a pontine hematoma which ruptured into the 4th ventricle. This patient also experienced acute lacunar infarction detected on diffusion-weighted MRI 5 months prior to the current presentation. The second case is an intraparenchymal hematoma in the left basal ganglion. The hematoma demonstrated increased size and mass effect with midline shift on the follow-up CT scan. MRI was performed in the first and DSA was performed in the second case and no underlying vascular or tumoral pathology was detected. The urine toxicology screen was positive for cocaine in both patients. Our aim in this report is to remind that stimulating drugs should be suspected when cerebral stroke is detected in young patients.
