Tube Guided Surgery for Lumbar Far-Lateral Disc Herniations

Surgery for disc herniations situated lateral to the foramen should not be performed
by facetectomy, since it may cause low back pain due to instability caused by facet
joint destruction. Mucle splitting approaches lateral to the midline if done by
classical techniques may also cause postoperative pain due to extensive muscle
dissection. For those reasons microscopic and endoscopic techniques have been
introduced. This paper describes on of these approaches using tube retractors by
endoscope or microcope.Tube guided surgery allows a 1.5 cm incision, no injury to the muscles, and use similar instruments that is accustomed by many spine surgeons. Table mounted retractor permits different viewing angles. By using this technique a day-surgery may be applied with additional advantages like short hospital stays and quick return to the work.
