Epidural Hematoma in an 8-Hour Old After Traumatic Birth: A Case Report

Traumatic epidural hematoma in infants is a rare entity. Its incidence increases when
the fetus is traumatized while passing the narrow birth canal, particularly in nullipar
mothers. The physical examination of an 8-hour old newborn after a prolonged
delivery indicated cephalic hematoma in the right temporoparietal and left
parietooccipital regions. Cranial computer tomography revealed 3 x 1.5 cm epidural
hematoma under the skull fracture in the right temporoparietal region neighboring
the cephalic hematoma. The hematoma was surgically evacuated. Head traumas
during birth are common in the newborn period. Epidural hematoma is a rare
condition because of specific cranial structure of the newborn, the dural adherenence
and dural arteries are not embedded into the skull bones. Clinical symptoms are
various. Surgical or conservative treatment options are controversial. Needle
aspiration should be the primary choice of treatment in cases with accompanying
cephalic hematoma for whom conservative treatment is ruled out. Hematomas
larger than 1 cm and hematomas that cannot be aspirated should be surgically
evacuated.
