Progressing microsurgical techniques, neuroanesthesia and intensive care applications have markedly decreased the mortality due to secondary injuries in head traumas. For all that, the severity and features of the injury sometimes render impossible a life saving surgery. A mortal case of orbitocranial injury is presented in this report. Surgical management of the case is extremely difficult and it is not certain whether there is an approach giving life chance. In these kind of cases, it may be required to introduce the approaches overseeing the individual properties of the case beyond the general principles and acknowledgment.
