The Effect of the Arachnoid Plane on Meningioma Surgery

Removal of the tumour including removal of the origin of the lesion has primary importance in meningioma surgery and the prevention of
tumor recurrences These tumors are usually encapsulated and dissecting them from the surrounding nerve and vascular structures is considered
easy. However, this is less true for more than half of the tumors. Preserving the complete circumference of the arachnoid layer around the
tumor is very important in tumour surgery without neurological damage. Tumor invasion into the pia and cortical layers is seen in cases with
a defective arachnoid layer. In this case, complete tumour removal can be complicated by ischemic/venous infarction due to dissection under
the pia and vascular structures. Preoperative computed tomography and magnetic resonance imaging are helpful in detecting the presence
of edema around tumors while angiography in defining the meningeal vasculature instead of the pial-cortical vasculature is helpful as a guide
showing absence of the arachnoid plane.
