Kayseri Eğitim ve Araştırma Hastanesi, Beyin ve Sinir Cerrahisi Kliniği, Kayseri, Türkiye The basic image of hemorrhages connected with aneurysms in brain tomography is subarachnoid hemorrhage although intraventricular hemorrhage, intracerebral hemorrhage and less frequently subdural hemorrhage may be present. These accompanying intracranial hemorrhages are associated with a poor prognosis and high mortality. Ruptured aneurysms which present with intraventricular hemorrhage and/ or intraparenchymal hematoma without subarachnoid hemorrhage are much less frequent. We present with the relevant literature a ruptured anterior communicating aneurysm that presented with intraventricular hemorrhage but without subarachnoid hemorrhage. We believe that further examination regarding vascular lesions should also be performed in non-traumatic pure intraventricular hemorrhages and that a larger percentage of ruptured aneurysms can be detected thanks to improvements in current radiological techniques.
