3İstanbul Üniversitesi, Cerrahpaşa Tıp Fakültesi, Patoloji Anabilim Dalı, İstanbul, Türkiye Although possesing similar histopathological features with classical meningiomas, primary intraosseous meningiomas are distinguished by their intradiploic localisation. These lesions are rare and frequently present with local swelling and headache. Atypical presentation is even rarer and the differential diagnosis-treatment process can be confusing. In this report we present the diagnosis and treatment process of a 55 years old male patient harbouring an intraosseous meningioma that was initially considered as calvarial metastasis. Intraosseous meningiomas should be included in the differential diagnosis of calvarial masses even in the absence of local swelling and pain symptoms. These cases should be meticilously investigated in the preoperative period and the treatment should be focused on total resection if possible. Adjuvant treatment should be considered in case of subtotal resection or in atypical or malignant intraosseous meningiomas and the patients should be followed up in frequent intervals.
