Etlik İhtisas Eğitim ve Araştırma Hastanesi, Nöroşirürji Kliniği, Ankara, Türkiye Acute subdural hematoma is one of the most common clinical entities encountered in daily neurosurgical practice and normally appears as a cresenteric collection of blood on CT scans. However, in rare conditions, some SDHs may have a lentiform shape, mimicking acute epidural hematoma. While the mechanism of this aspect is not well understood, most theories blame on the adhesions and the blockage of the subdural space. In this article we describe a 79 years old female patient with an acute subdural hematoma mimicking a lentiform epidural hematoma radiologically and disscuss the differential features of these two entities on CT scans. The suspicious point in our case was the discordant clinical status with such an epidural hematoma. In the modern era that we live, diagnosis are usually made on the basis of high tech radiological examinations. With this case we emphasize the importance of the clinical evaluation and physical examination.
