Distal Posterior Inferior Cerebellar Artery Aneurysm: Case Report

A 70-year-old male was admitted with a diagnosis of subarachnoid
hemorrhage. There were no pathological findings on digital substraction
angiography. He was hospitalized 6 weeks after the hemorrhage when magnetic
resonance imaging revealed a 10 mm lesion on the course of the posterior
inferior cerebellar artery. Digital substraction angiography showed an 8 mm
aneurysm on the distal left posterior inferior cerebellar artery. The distal
posterior inferior cerebellar artery aneurysm was clipped by a suboccipital
median approach. The patient was discharged on the postoperative 7th day
without any neurological deficit. Subarachnoid hemorrhages with an unknown
origin should be reassessed with magnetic resonance imaging and digital
substraction angiography as it may not be possible to make a definite diagnosis
with early investigations.
