Indications of Epilepsy Surgery and the Preparation of Epilepsy Patients

Uludağ Üniversitesi, Tıp Fakültesi, Nöroloji Anabilim Dalı, Bursa, Türkiye AIM: Seizures are poorly controlled despite current antiepileptic treatments in many people with epilepsy. Diagnosing drug-resistant epilepsy will facilitate referral for early consideration of epilepsy surgery.

MATERIAL and METHODS: Epilepsy surgery is a frequently applied treatment modality in order to improve quality of life. Appropriate patient selection is important for the success of surgical treatment. The primary objective of investigations is to identify the onset region responsible for generating the patient’s habitual seizures. The steps in all these examinations are typically carried out by a multidisciplinary team.

With the technical developments of video EEG monitoring and advanced structural and functional neuroimaging, epilepsy surgery is becoming safer and more eff ective. The most frequently applied surgical techniques at present include temporal lobe surgery, extratemporal neocortical resection, multilobar resection, hemispherectomy, lesionectomy, corpus callosotomy and multiple subpial transection.

CONCLUSION: Refractory epilepsy, the selection of candidates for surgery, electrophysiological, radiological and neuropsychological preparation and indications for surgery in epilepsy patients are reviewed in this article.
