This progressive disease mainly invades the liver, and develop tumor like lesions that may metastasise to various tissues (lung, spleen, kidney, pancreas, lymphoid tissue, bone, ovaries, suprarenal glands). Cerebral involvement is seen only in 1% of patients infected with this parasite. Only cerebral involvement without liver invasion may be seen in very rare cases. The diagnosis can generally be made via biopsy in the late phases of the disease. The first treatment option is surgical excision followed by medical treatment. We report a 20-year-old young female with intracranial parenchymal and leptomeningeal involvement who was operated for hepatic alveolar echinococcus. The related literature is reviewed.
