Endoscopic Endonasal Transsphenoidal Treatment of Acromegaly

Ankara Numune Eğitim ve Araştırma Hastanesi, Ankara, Türkiye Acromegaly is a chronic disorder characterized by excessive secretion of growth hormone and elevated insulin-like growth factor-1 levels, usually due to a pituitary adenoma. It can cause serious mortality and morbidity if not treated. The first treatment option is surgery and endoscopic endonasal transsphenoidal surgery (EETS) has now become the procedure of choice. EETS has become popular due to advantages such as a better field of view, less nasal trauma, ability to access parasellar regions with the angled endoscope, and good results following tumor resection. The post-treatment remission criteria for acromegaly have gradually become more demanding and currently include a random GH level of < 1 ng/ml, post-oral glucose tolerance test GH level < 0.4 ng/ml and a normal IGF-1 level for age and gender 3 to 6 months after surgery. These more strict criteria have shown that surgical treatment needs to be very effective and aggressive. EETS is an effective and safe method for acromegaly treatment.
