Neural tube defects (NTD) are multifactorial midline anomalies that occur during the neurulation process in embryogenesis. Myelomeningocele is an important anomaly that may indicate the necessity of emergency surgical intervention. There are a small number of studies on twins with NTD and the diff erence in incidence in comparison to single births is insignificant. Concordance is about 7% in monozygotic twins and below 4% in dizygotic twins. While risk factors such as genetic basis, folate deficiency, use of antiepileptic drugs, maternal diabetes or obesity have been determined in the epidemiology; there is often a multifactorial disease involving a subset of the aforementioned. In this article, we discuss a rare case of dizygotic twins with myelomeningocele, born to a mother with maternal diabetes and with no other risk factors whatsoever.
