Cerebellar agenesis is very rarely seen, it can cause death in neonatal period and serious paralysis, also it can be seen as asymptomatic in adult period. The probability of its diagnosis increased with the advance of radiological tools. Patients with cerebellar agenesis may have the disturbances of motor, mental condition, cerebellar coordination or not any. Additional congenital disturbances generally are seen with Dandy-Walker malformation and Chiari malformation. We believe that these patients’ differential diagnosis is important for the evaluation of further diagnosis and treatment. We evaluated 32-years old male patient, complaining headache investigated with radiological screenings showed primary isolated total cerebellar agenesis, with literature.
