Internal carotid artery (ICA) aneurysms are treated by surgical or endovascular approaches. Pterional craniotomy is the most commonly used approach in the surgical treatment of ICA aneurysms. The most important factor affecting success in the surgical treatment of bleeding ICA aneurysms is the timing of the treatment. It should not be forgotten that the neurosurgeon who applies surgical treatment for an ICA aneurysm should have experience and knowledge about the anatomical structures as well as their variations. Cerebral aneurysms are vascular lesions with high mortality and morbidity. Their endovascular or surgical treatment should be planned as soon as possible by experienced neurosurgeons.
