Surgical Approach to Far Lateral Lumbar Disc Herniations

However, far lateral disc herniations (FLDH) make up a minor portion of the indications. Clinical presentations include severe radicular pain with the femoral stretch test and Lasegue test and neurological deficits are seen in the majority of these patients. Magnetic resonance imaging (MRI) is the most useful neurodiagnostic evaluation for FLDH. Because of their lower incidence and the different anatomical approach to the lateral interpedicular compartment to avoid damaging the nerve or facet joint, most spine surgeons find these cases challenging. There are various surgical approaches to FLDH and surgeons make a decision according to the anatomical location of the disc herniation and the clinical condition of the patient.
