Vascular malformations of the pineal region are uncommon lesions and approaching this region is still challenging because of the deep localization and close relationship with important neural and vascular structures. However, in the last few decades, endovascular techniques and radiotherapy have become increasingly popular for the treatment of these lesions. The microsurgical option is still valid when these techniques are not feasible or fail. The main vascular malformations of the pineal region are vein of Galen malformations, arteriovenous malformations and cavernous malformations. The most common approaches used for the pineal region are the supracerebellar infratentorial and occipital transtentorial approaches.
