THE ROLE OF INTRAOPERATIVE ULTRASONOGRAPHY IN DETERMİNİNG THE INTRACRANIAL TUMOR RESECTIONS

Purpose: To detect the efficacy of intraoperative sonography in determining the localization of intracranial tumors and to identify the sonographic features of postresection cavity borders in determinig the extent of resection.

Methods and Materials: After the resection was over, the cavity borders of 32 tumors were examined with an intraoperative probe. Thickness of the echogenic rim and the presence of intraoperative hemostate (Surgicel) were correlated with preoperative and postoperative Magnetic Resonance images. 5 mm or less thickness of echogenic rim was considered as normal.

Results: In ali cases, the tumor location, borders and its anatomical relations were well depicted in sonographic images. Kappa value fo?ragreement on the residue detection was +0.72. There were four cases in whom magnetic resonance images showed residue despite a negative sonography: The reasons for mismatch were extensive edema or Surgicel along the cavity borders in three cases, and cystic component in the vicinity of cerebrospinal fluid in one case.

Conclusion: Intraoperative ultrasound is a cheap, harmless and effective tool in detecting the tumor location, borders and antomical relations. Sonographic findings should be evaluated carefully in cases with extensive pericavitary edema, cystic component in the vicinity of cerebrospinal fluid and extensive application of intraoperatve Surgicel.
