Treatment Strategy in Anaplastic Meningiomas

Most of them are grade I benign tumors, but 2% of them are grade III meningiomas with malignant characteristics. Most related studies are done by grouping them together with grade II meningiomas as "high grade meningiomas" due to the low numbers. Data regarding their natural and history, clinical follow-up, pathology, genetics and epigenetics are scarce, but have been increasing recently. MRI is the gold standard for their diagnosis and treatment, but there is still no standard protocol for preoperative prediction of pathological grades. Surgical resection followed by adjuvant radiotherapy is performed in their treatment as the standard protocol. Reoperation and radiotherapy should be preferred for recurrent cases, but radiosurgery, Lu-177-DOTA-TATE, and chemotherapy can be advised for inoperable cases.
