Cervical spine alignment plays an increasingly important role in the surgical planning, preoperative evaluation, and surgical treatment of various cervical spine pathologies. The complex and susceptible nature of the cervical region makes it susceptible to malalignment that may inevitably require surgical intervention, and to various spinal pathologies that begin with this malalignment. Numerous articles have been published showing that cervical spine malalignment is associated with a poor clinical outcome and poor quality of life. Currently, the surgical indications for correcting cervical alignment are not well defined and there is no established standard to address the amount of correction desired. Careful preoperative evaluation of cervical spine alignment can assist spine surgeons in clarifying surgical goals, determining the appropriate surgical approach, and ultimately achieving a biomechanically sound spinal structure that optimizes clinical outcome.
