INTRACRANIAL MULTIPLE ANEURYSMS

In this retrospective study which covers a period of 20 year at İstanbul School of Medicine Neurosurgery Department, the incidence of multiple intracranial aneurysms was found to be 14.3% (103/722). Sex distribution was equal and the mean age was 46.9.

Two hundred-forty five aneurysms were found in the 100 patients who admitted with subarachnoidal haemorhage and three others with other causes. 92.2% of the aneurysms were located in the carotid, and 7.8% in the vertebro-basilar (VB) system. Middle cerebral artery, anterior communicating artery and internal carotid bifurcation aneurysms were the most commonly found and ruptured aneurysms.

94 patients (91.2%) were surgically treated. Though in recent years surgical mortality rate was reduced to 14.7% from 21.3%, it is still far more higher than the 3.4% rate of surgical mortality of single aneurysm cases. Especially VB system aneurysms are of greater risk.

Advances in the neuroradiological procedures will improve the rate of determination of the site of rupture and reduce morbidity and mortality rates, as well as the improvements in surgical technique and early surgery.
