Brain abcess due to Mycobacterium tuberculocis (M. tuberculosis) is a rare disease even in areas endemic for tuberculosis. Medical therapy and surgical excision are the treatment options. Recent reports conclude that the outcome of the disease is favourable with stereotactic aspiration of the lesions. We present a patient with brain abcess due to multi-drug resistant M. tuberculosis. The abcess of the patient was drained three times via stereotactic aspiration and finally totally excised. Minor antituberculosis drugs were used for medical treatment of the patient.
