THE CAVERNOUS INTERNAL CAROTID ARTERY BYPASS USING SAPHENOUS VEIN GRAFT: CASE REPORT

Dokuz Eylül Üniversitesi Tıp Fakültesi Nöroşirürji Anabilim Dalı, İzmir A 48-year-old woman with diplopia who attended to our clinic was diagnosed as right cavernous sinus meningioma by radiological examinations and she was operated. Cavernous sinus was opened by Dolenc technique and tumour tissue, which was located at paramedial and Parkinson triangles, was excised. During this procedure carotis artery was ruptured. Bleeding localization couldn't be detected because of the cranial nerves and as a result primary repair couldn't be done. However, the flow of internal carotid artery was stopped when packing was done, C6-C2 end to side anastomosis was performed with saphenous vein graft under barbiturate coma. The patient was discharged with right opthalmoplegia. In this paper, the indications of bypass procedure with saphenous vein graft and the surgical techniques was discussed with our case and literature. In surgical procedures targeting skull base and cavernous sinus, in case of ICA repair or bypass during surgery, it is essential to evaluate ICA prior to surgery by angiography or baloon occlusion test.
