Osteoporosis is a skeletal disorder characterized by the diminished density of normal mineralized bones The reduced bone density leads to a decrease in mechanical resistance and causes bone fracture. The factors which cause this disease should be examined as primary (age related, postmenopausal) and secondary (endocrine and neoplastic disorders, drugs and chromosomal anomalies) factors. The main problems that arise for the spine affected with osteoporosis are instability caused by deformity, and neurological complications. Most of the cases are observed in the over aged group besides the fact that clinical manifestations always appear after a long latent period. Bone densitometry and X-ray are the most significant tools for diagnosis. Treatment has to commence with the prevention of the disease in the risk group. The therapy for actual osteoporosis that has already started to form must firstly begin with medical treatment. Surgical treatment should only be applied to patients with progressive deformity and neurologic findings. The aim of surgery should be to stabilize the vertebral column rather than to correct deformity. Anahtar Kelimeler : Compression fracture, instrumentation, osteoporosis, spine
