ENDOSCOPE ASSISTED TUMOR SURGERY

Kocaeli Üniversitesi Tip Fakültesi Nöroşirürji Anabilim Dalı, Derince, Kocaeli Objective:The benefits and the advantages of endoscope-assisted microsurgery in the intraoperative management and controlling of the resection procedure during intracranial tumor surgery are discussed.

Methods: During the years 1997-2001, we performed 13 endoscope-assited tumor surgeries by combining rod lens endoscopes with the operating microscope. In endoscopeassisted surgery, 3 ipsilateral, 1 transventricular, 5 transbasal and 4 intracystic endoscopic approaches were performed.

Results: The lesions treated with endoscope-assisted surgery comprised of 6 pituitary adenomas, 4 cystic intraparanchymal glial tumors, 2 epidermoid tumors and 1 metastatic clival tumor. No severe complication due to endoscopic intervention was occurred. In general, during endoscopy a narrow surgical corridor was maintained and no additional trauma was imported to the surrounding structures.

Conclusion: The authors conclude that endoscope-assisted microsurgery is of significant benefit and supportive method in tumor surgery, by performing minimum exposure craniotomies, avoiding additional resection of dura, bone and brain tissue, and achieving better visualizatjon of deep topographical structures and hidden areas which cannot be directly visualized through the viewing angle of the microscope.
