S.B. Ankara Eğitim ve Araştırma Hastanesi Nöroşirürji Kliniği, Ankara Presence of intracerebral hematoma due to aneurysmal rupture points out a poor prognosis. In such cases, a hematoma drainage without a surgical approach to the aneurysm carries high mortality because of rebleeding. In this article, we report an eighty-one year old patient with herniation secondary to intracerebral hematoma. An urgent surgery by clippage of the established aneurysm in the bifurcation of middle cerebral artery with sylvian dissection was performed following the hematoma drainage. The procedure was successful. Thus, in cases of hematomas which points out an aneurysm because of its location, a surgical interference to the aneurysm along with the hematoma might be a life-saving procedure as in our case Anahtar Kelimeler : Intracerebral hematoma, Aneurysm, Early operation
