In this article, we reported an iatrogenic epidural inflammatory reaction developed after lumbar disc surgery acting like spinal epidural empyema. It is the clinic, progress and the result that make our case distinctive. Spinal epidural empyema was not considered primarily when clinic, laboratory and culture analysis was taken into account. However, the possibility of spinal epidural empyema couldn't be eliminated once we considered the lumbar surgery, interpretation of lumbar MRI and macroscopic findings that arose during the surgery. Spinal epidural empyema is a condition that may develop after lumbar disc surgery. Recovery can be achieved by early diagnosis and antibiotic treatment after surgery. In our case, we differentiated iatrogenic epidural inflammatory reaction and spinal epidural empyema. The condition developed in our case was tried to be explained by two hypotheses.
