The authors aim to present a case of chordoid meningioma with atypical radiological and clinical findings. Fifty-year-old male patient presented with hydrocephalus. His cranial radiological evaluation revealed diffuse contrast enhancing lesions of leptomeningeal area. Pathological evaluation revealed chordoid menengioma. All of the previously reported cases of chordoid meningiomas were solitary mass lesions that could easily be attributed to intracranial mass lesions as benign or malign, according to their radiological appearance. However, the present case had atypical magnetic resonance imaging findings with its diffuse leptomeningeal spread leading to misinterpretation of the radiological data. To the best of our knowledge, this is the first reported case of a chordoid meningioma diffusely involving the entire leptomeninx.
