Safe Clipping Technique of Middle Cerebral Artery Aneurysms with Superior Mobilization of Aneurysm Dome

Ondokuzmayıs Üniversitesi, Tıp Fakültesi, Beyin ve Sinir Cerrahisi Anabilim Dalı, Samsun, Türkiye AIM: Surgical clipping of middle cerebral artery aneurysms is the primary treatment method. The dome projection is among the factors affecting the surgical strategy.

MATERIAL and METHODS: The patients operated between the dates of 2010-2011 because of middle cerebral artery aneurysm were reevaluated in terms of the dissection technique of aneurysm dome and clipping technique. The specific surgical technique was described in the cases with inferiorly projected aneurysm. This technique was summarized as inferior wall dissection and superiorly mobilization of aneurysm dome.

RESULTS: Sixty-seven cases were included in this study. Thirty (45%) cases were male and 37 (55%) were female. The aneurysm dome projected to the insular cortex in 24 (36%) of the cases. Intraoperative aneurysm rupturing was seen in three cases (12%). Temporary aneurysm clips were used in 9 (37%) cases.

CONCLUSION: Safe clipping is possible with the dissection of inferior wall of the aneurysm from the insular cortex and superiorly mobilizing the aneurysm dome. Anahtar Kelimeler : Middle cerebral artery aneurysm, Safe clipping, Aneurysm
