Granulocytic sarcoma is a malignant tumor consisting of immature myeloid cells located at an extra-medullary site. It is generally seen during the course of leukemia but they can rarely present in a nonleukemic patient or previously treated leukemic patient without medullary relapse. Although it can occur anywhere in the body, cerebral dissemination is unusual. Granulocytic sarcoma involving the cavernous sinus is an extremely rare entity with a limited number of reported cases. The authors present interesting clinical and radiological features of such a case manifesting with trigeminal neuralgia. Pathogenic mechanisms for central nervous system dissemination, the challenges and important points in the differential diagnosis are also discussed.
