Removal of the amygdala and hippocampus through a transsylvian approach is one of the selective surgical procedures for the treatment of intractable mesial temporal lob epilepsy. Selective surgical removal of these structures determined as the epileptogenic focus is based on avoiding trauma to the adjacent, normal and healthy temporal areas by performing pure lesionectomy. In this report, we tried to describe the selective amygdalohippocampectomy technique via the pterional transsylvian approach described by Yaşargil.
