Serbest Hekim, İzmir, Türkiye AIM: Surgery of the masses located in functional areas such as motor cortex and speech areas leads to high morbidity. Awake craniotomy is a safe method for excision of these lesions.

MATERIAL and METHODS: 801 patients who were operated on between the years of 1999-2014 were retrospectively evaluated and the results are discussed.

RESULTS: Low morbidity and mortality was observed in the patients operated on with this method.

CONCLUSION: Awake craniotomy is used for resection of lesions located in and around the functional cortex or close to the tracts of the internal capsule or arcuate fasciculus. With this method, the risk neurologic deficits is reduced to a minimum. A total of 801 patients who were treated with this method between the years of 1999 and 2014 were evaluated.
