Intraoperative ultrasonography (IOUS) has recently gained wide acceptance in neurosurgery. Here, we aim to highlight the importance of IOUS in rendering maximum resection with minimal morbidity possible in spinal space-occupying lesions. We present a case of spinal hydatid cyst located in the thoracolumbar area and discuss the eff ect of IOUS application on surgical morbidity. A 19-year-old male patient was admitted to our clinic with complaints of diff iculty walking and loss of urinary/defecation control, especially for the last two days. Multiple cystic lesions at the levels of T-12 and L-1, L2 were encountered on spinal MRI. The patient underwent surgery and the cystic lesions were totally resected under the guidance of IOUS. His neurological functions significantly improved postoperatively. He was discharged on postoperative day 5 with a recommendation of long-term physical therapy.
