Pituitary Macroadenomas

Although pituitary adenomas are considered as benign tumoral lesions, clinical morbidity is high in cases of neurovascular compression and hypopituitarism. In macroadenoma cases with the presence of cavernous sinus infiltration and partial or total opthalmoplegia, compression of chiasm, presence of apoplexia in T2 weighted MRI, and 3rd ventricle compression with panhypopituitarism; surgery should be planned as soon as possible. The success of surgical treatment greatly depends on the selection of the surgical approach. Complete removal of the tumor using the best and most appropriate surgical method in the first intervention must be the ultimate goal. To avoid the complications of endoscopic macroadenoma surgery, the long and hard learning curve of endoscopic surgery must be considered. Multidisciplinary team work must be adopted, preoperative evaluation of neuroendocrinological and neuroradiological workup and individual sellar and tumor measurements must be performed.
