A Case of an Alien Hand Syndrome due to Intracranial Meningioma

Alien hand syndrome is an interesting and rare neurological finding characterized with one-sided involuntary movements and/or
perception disorders of the hand, which develops due to an intracranial pathology. It is mostly seen in post-stroke, corticobasal
degeneration, multiple sclerosis, Creutzfeldt Jakob’s disease or post-callosotomy patients. Alienation or the loss of proprioseptive
awareness may seen on the affected hand. A forty-four years old male patient presented with estrangement and loss of control
for two weeks on his left hand. He also had a weakness on the left side of his body for last two days. His neurological examination
showed slight left hemiparesis, abnormal cerebellar findings, Romberg’s sign and involuntary movements on left hand. Cranial
magnetic resonance imaging revealed a parafalxian extra-axial mass lesion in 6x5x5 cm size adjunct to bilateral premotor gyri
and compression of the splenium of corpus callosum. Patient underwent surgical excision of the mass and all his pathologic
neurological findings disappeared gradually at one week after surgery. Alien hand syndrome is a rare neurological phenomenon with
unclear etiology and pathogenesis. Majority of the alien hand syndrome cases have been reported with frontal, parietal or corpus
callosum lesions. Alien hand syndrome due to an intracranial mass is extremely rare.
