The Management of Posterior Fossa Metastasis

It is often believed to be a more immediately lifethreatening complication than brain metastasis in other locations. Headache is a common presenting symptom, is more common with multiple metastases or with posterior fossa lesions and may be mild. Open microsurgery is effective in improving the Karnofsky performance score (KPS) in patients with cerebellar metastases, despite a high complication rate. Surgery followed by radiotherapy resulted in significantly improved median survival compared with radiotherapy alone. Relative contraindication of radiosurgery would be posterior fossa metastases associated with cerebrospinal fluid flow obstruction and hydrocephalus. Leptomeningeal disease (LMD) was associated with rapid and considerable decline in the quality of life; it does not influence the overall survival.
