The Mini-open Transpedicular Discectomy

Symptomatic thoracic disc herniations are rare in the general population. Thoracic
disc herniations can be surgically treated if caused significant radicular pain and
severe, progressive myelopathy with a number of different open techniques and
approaches. Unfortunately, to obtain adequate visualization and working space,
these approaches require extensive tissue and muscle dissection in excess of the
actual area of surgical interest. Recent technical advances that minimize surgical
approaches have become popular in the treatment of spinal disorders especially in
the lumbar spine, but little data has been published regarding such approaches
(thoracoscopic techniques, minimally invasive tubular approaches) in the thoracic
spine. Thoracoscopic techniques have also been pioneered, but require a steep
learning curve. The mini-open transpedicular discectomy can be used to treatment
of appropriate thoracic disc herniations without require a step learning curve. This
technique do not use for midline, large, adherent to and erode the dura, calcified
herniated discs causing obliteration of the spinal canal like as open transpedicular
discectomy. This technique provides a safe method to perform thoracic discectomy
with minimal paraspinal soft tissue disruption and less blood loss during surgery.
