Rapid Development of a Large Pseudoaneurysm Following a Clipped Anterior Communicating Artery Aneurysm

We present an unusual case of a large, new aneurysm formation that developed and ruptured within 15 days following microsurgical clipping of a small anterior communicating artery (AComA) aneurysm. This secondly-developed aneurysm possessed all the characteristic angiographic features of pseudoaneurysms such as significant morphological changes in a short period of time, globular shaped aneurysmal sac without a neck, and delayed filling and washout of contrast agent from the aneurysm. Local fragility of the vascular wall due to the clip edge, microscopic residual neck, and damage of the vascular wall during the first surgery might have been the possible factors related to the development of the pseudoaneurysm.
