Degenerative spine poses a serious risk of morbidity in elderly patients. Intraoperative neurophysiological monitoring (IONM) is a diagnostic method that is increasingly used in modern spinal surgery and can provide a significant increase in surgical safety. It is important to know the neurological status of the patients during surgery. Spontaneous maneuvers and manipulations during surgery allow the best surgical result to be achieved. IONM is a diagnostic method that is increasingly used in modern spinal surgery and can provide a significant increase in surgical safety. With its correct use, it contributes to the prevention of irreversible neural damage with preventive or corrective applications and maneuvers by providing instantaneous information flow to the surgeon about the functional status of the spinal cord and nerve roots during the operation. There is increasing evidence that the use of IONM is effective in detecting adverse neurological outcomes during the surgery of cervical spinal degenerative diseases; however, since prospective studies with high levels of evidence are still insufficient, its use today depends on the surgeon"s preference and local institute facilities.
